Online Forms

Some of our online forms require students to set up a delegate so that the delegate may complete their section (such as a parent tax or a dependent verification worksheet) electronically. Please check https://servicecenter.fsu.edu/s/article/Delegated-Access for details on how to set-up delegate access and choose ‘Financial Aid – All’ to give them the appropriate permissions.

Several of our most frequently used documents are available for you to complete online through your student portal at my.fsu.edu and we would encourage you to use that option. These forms have a link to the my fsu student portal.

Some documents, such as the Verification Worksheet and the Tax Forms may be completed electronically by navigating within Student Central to: Financial Aid > Select correct Aid Year > Documentation Requested > Select available Document Name.

Electronic signatures are not acceptable at this time, however, wet signatures or copies of wet signatures are acceptable.
