You may submit a completed form USING THE FOLLOWING METHODS:

  • Noles File transfer (NiFTy):  https://nifty.fsu.edu/

    • If using NiFTy, please note: use the email financialaid@fsu.edu.  This is the quickest and most secure way to submit your documents.  Encrypting the documents you send via NiFTy is not necessary, but if you choose to encrypt please send the passcode to financialaid@fsu.edu in a separate email.

Online Forms

  • Some of our online forms require students to set up a delegate so that the delegate may complete their section (such as a parent tax or a dependent verification worksheet) electronically. Please check https://servicecenter.fsu.edu/s/article/Delegated-Access for details on how to set-up delegate access and choose ‘Financial Aid – All’ to give them the appropriate permissions.

  • Several of our most frequently used documents are available for you to complete online through your student portal at my.fsu.edu and we would encourage you to use that option. These forms have a link to the my fsu student portal.

  • Some documents, such as the Verification Worksheet and the Tax Forms may be completed electronically by navigating within Student Central to: Financial Aid > Select correct Aid Year > Documentation Requested > Select available Document Name.

  • Electronic signatures are not acceptable at this time, however, wet signatures or copies of wet signatures are acceptable.

Personal Circumstance Verification

This form is for students who answered 'Yes' to at least one of the dependency-related questions in the Personal Circumstance portion of the FAFSA Application. Formerly known as Independent Status Verification.

This form covers the following dependency-related circumstances:
  • Active Duty
  • Veteran Status
  • Student has Dependents
  • Emancipated Minors
  • Unaccompanied Youth/Homelessness
  • Students in Legal Guardianships
  • Orphaned Students
  • Students who are Wards of the Court
  • Students in Foster Care

Unusual Circumstance

This form is for students who may have conditions that justify adjusting a student’s dependency status, commonly known as a dependency override.

Circumstances that May Warrant a Professional Judgement:
  • Abusive/Neglectful home environment.
  • Severe estrangement of family.
  • Abandonment
  • Incarcerated parent.
  • The student is an unaccompanied youth.
  • The student is homeless or at risk of homelessness.
  • The student is now married.
  • Other
Circumstances that Do Not Warrant a Professional Judgment Adjustment:
  • Contributors (Parents) refuse to contribute to educational costs.
  • Contributors (Parents) are unwilling to provide information on FAFSA or for verification purposes.
  • Contributors (Parents) do not claim the student as a dependent for income tax purposes.
  • Contributors (Parents) income is too high to qualify for need-based aid.
  • The student demonstrates total self-sufficiency.

Special Circumstance Forms

Application (Special Circumstance)

This form is for students who may have a special circumstance that is affecting their SAI (Student Aid Index).

Circumstances which might be considered unusual may include (but are not limited to) the following:

Income Reductions
  • Unemployment/Change in Employment
  • Retirement
  • Disability
  • Loss of Benefits and/or Untaxed Income
  • Other situations not listed above
Divorce/Separation
  • This will be used if parents of dependent students divorce or separate after completing the FAFSA
  • OR
  • If the student is independent and divorced, or separated from their spouse after completing the FAFSA.
Death of a Parent/Spouse
  • To be filed only if initial FAFSA data included deceased parent (or student's spouse) in income or household.
Non-Elective Dental/Medical Expenses
  • Only the portion of expenses which exceed 11% will be considered an unusual circumstance.

Cost of Attendance Adjustment

This form is for students who have unexpected costs related to their educational expenses.

These unexpected costs may include:
  • Cost of a Computer
  • Required Item by Department
  • Housing Expenses
  • Dependent Care Expenses
  • Emergency Medical/Dental Expenses
  • Tuition Expenses
